Important notification from the Embassy of Lebanon in Ottawa

The Embassy of Lebanon would like to inform all Lebanese living in Canada that starting June 1st, 2011 and at the request of the International Civil Aviation Organization (ICAO) and after the Lebanese authorities were informed that more than thirty countries are now reluctant to grant visas or renewing residency permits for holders of Lebanese burgundy passports, for these reasons the Embassy asks its citizens to apply through its consular section for obtaining a new passport (Biometric-Color Blue) issued exclusively by the Directorate of General Security in Lebanon.

Applications for new passports will be sent to Lebanon by diplomatic pouch and returned to the Embassy in the same way.

Getting a new passport will take at least one month from the date of submission of the application to the Embassy.

N.B. The Embassy of Lebanon is able to renew blue passports to its citizens, who left Lebanon during the evacuation that took place in July 2006, and have not been stamped by the Directorate of General Security, after receiving approval from the competent authorities for applications’ renewals for five years or after presenting a new Judiciary Act (dating from the year 2007 and over) for application for one year only.

Getting a new passport

1. Complete the form including the signature and stamp of the concerned person at the Embassy in front of the Consular Officer.

In case you reside outside of Ottawa, you need to send your full address (by fax or e-mail) in order to receive the application; then you should fulfill, affix your right thumb and sign the application in front of a notary public at the designated places.

2. Completing the passport’s form:

The passport form has a computer number and can not be photocopied or even printed and must be filled with a blue ballpoint pen or printed, provided that the writing is clear and readable.

It is forbidden to fold the form and attach with staples to other papers; other documents are enclosed with a clip.

N.B: the sample application form is used as an example and may not be presented as formal application.

a. Completing details:
Complete details of the person in Arabic according to the Extract of Individual Registration or Identity Card, and in foreign languages (Latin letters) according to his passport if available, and adding a copy of a foreign document (residence permit, driving license, passport…) for any change of details in the foreign language upon personal request of the applicant.

b. Marital Status (for women only):
Add x to the proper place (single, married, divorced, widowed) and write the name of the spouse for the married.
Addition of family status depends on the willingness of the person by adding an x in front of “I want” or “I do not want”.
Addition of the Spouse’s name on the form should be in Arabic and foreign language.
The addition of family status is required if there are children who accompany their mother on her passport.

c. Address:
Add complete and clear address in Lebanon in Arabic and in the country of residence in Arabic or foreign language, as well as phone numbers and email address.

d. Adding children:
You can add a maximum of three children as it is on the passport of the father or the mother (can not add a child on the passport of his brother).
The age of the child being added must not exceed fifteen years.
Children aged 15 and older should get personal passports and their applications must be signed by their fathers, legal guardians and lawyers (compulsory status for children minors under 18).

e. Permission to travel:
This permission shall be signed by the father and mother (jointly) for children under 7 years and the father only for children between 7 and 18.

f. Profession:
The profession is added to the form upon request of the concerned person after providing a document proving it (belonging to the orders of engineers or physicians in Lebanon).

g. Number of old passport:
If you would like to change your passport, you will need to register the number of the old passport with a photocopy of it to add to the application.
Once the new passport is given to the applicant by the Embassy, the latter will proceed to cancel the old passport and submit it to the concerned person.

h. Application type:
Put an x at the proper place (New passport, lost passport, normal change…)

3. Requested documents (to be added to the form):

3.1 Individual Passport:

a. Recent extract of individual registration (Ikhraj Kayd Fardy date of issuance does not exceed 5 years) or Lebanese ID (new one computerized) or a photocopy of passport of the person issued in Lebanon after the year 1990: Only notarized copies of these documents are acceptable.

b. A copy of the passport that the applicant intends to replace.

c. Two recent color photos (Size 4,3 x 3,5 cm not drawn from another photos) showing full details of the face with a white background.

d. Self-addressed stamped, Express Post, envelope with phone number (to send you back the original documents and passport).

e. The amount of:

40 CAN$ for one year
200 CAN$ for five years
In person: cash
By mail: Money order payable to the Embassy of Lebanon.

3.2 Family Passport:
In addition to the above documents, the applicant should have in possession:

a. Two recent color photos of the child (according to the specifications mentioned above-clause c). Paste the child’s photo in the space provided for that. (1st child in the middle, 2nd child to the left and the 3rd to the right).

b. An extract of family registration including the names of accompanying minors to be added to the passport.


Passport Renewal

I - Fill out and stamp your right thumb on 2 identification cards at the Embassy in front of the Consular Officer.

In case you reside outside of Ottawa you need to send a self-addressed stamped envelope and the Embassy will mail you 2 identification cards. You should stamp your right thumb in front of a Notary Public and attach the following statement:

"Before me, a notary public, in and for County of ________________, personally appeared Mr./Mrs.____________________________________, holder of Lebanese Passport or Lebanese ID (Tazkarat Al-Hawiya or Ikhraj Kayd) No._____________________, issued Date:________________________, Place:__________________, who signed and affixed his/her right hand thumb on the annexed identification cards. In testimony whereof, I have hereunto subscribed my name at (County) ______________________________, (date)__________________(Seal)"

II -

A-Burgundy Passport

Fill out a Passport Application Form and present it to the Embassy with the following documents:

  1. Actual passport with 1 photocopy of the first three pages.
  2. Recent Personal civil registration (Ikhraj Kayd Fardy) or the new Lebanese ID (Hawiya) with 2 photocopies.
  3. The 2 fingerprinted identification cards.
  4. 3 photos (Passport size).
  5. For under 18 years of age, a Consent Letter signed by the father - Or for newborn up till 7 years of age, a Consent Letter signed by both parents Authorizing the child to obtain a passport
    • In person: at the Embassy in front of the Consular Officer.
    • By mail: in front of a notary public.
  6. The amount of $40.00 for one year, or $200.00 for five years
    • In person: Cash.
    • By mail: Money order payable to the Embassy of Lebanon.
  7. Self-addressed stamped, Express Post, envelope with phone number.

B- Computerized Blue Passport

Fill out a Passport Application Form and present it to the Embassy with the following documents:

  1. Actual passport with 1 photocopy of the first page.
  2. Recent (not exceeding 8 years) Personal civil registration (Ikhraj Kayd Fardy) or the new Lebanese ID (Hawiya) with 2 photocopies.
  3. The 2 fingerprinted identification cards.
  4. 3 photos (Passport size).
  5. For under 18 years of age, a Consent Letter signed by the father - Or for newborn up till 7 years of age, a Consent Letter signed by both parents Authorizing the child to obtain a passport
    • In person: at the Embassy in front of the Consular Officer.
    • By mail: in front of a notary public.
  6. The amount of $40.00 for one year, or $200.00 for five years
    • In person: Cash.
    • By mail: Money order payable to the Embassy of Lebanon.
  7. Self-addressed stamped, Express Post, envelope with phone number.

Replacing a lost passport

-The loss of a passport should be reported immediately to the local police that will issue a police report.

-A notice about the lost of the passport should be published in a local newspaper for three consecutive days.

-The Embassy can issue a new passport replacing the lost one only after authorization from the Directorate of General Security.

1- The requested documents:

a. A photocopy of the lost passport (better if available)

b. An extract of familial registration or two notarized copies of the document.

c. An extract of individual registration or the new I.D. card computerized or two notarized copies of these documents.

d. Fill out the form of a new passport in the same manner as paragraph I and amend article (h) only by putting an x at the proper place (Replace or lost passport).

e. Self-addressed stamped, Express Post, envelope with phone number.

f. The amount of:

40 CAN$ for one year
200 CAN$ for five years
In person: cash
By mail: Money order payable to the Embassy of Lebanon.


New or renewal of travel documents for Palestinian

I - Fill out and stamp your right thumb on 2 Palestinian identification cards at the Embassy in front of the Consular Officer.

In case you reside outside of Ottawa you should send a self-addressed stamped envelope and the Embassy will mail you 2 Palestinian identification cards. You should stamp your right thumb in front of a Notary Public and attach the following statement:

"Before me, a notary public, in and for County of ________________, personally appeared Mr./Mrs.____________________________________, holder of holder of travel document or Palestinian I.D. (Tazkarat Al-Hawiya) No._____________________, issued Date:________________________, Place:__________________, who signed and affixed his/her right hand thumb on the annexed identification cards . In testimony whereof, I have hereunto subscribed my name at (County) ______________________________, (date)__________________(Seal)"

II - Fill out a Travel Document Form and present it to the Embassy with the following document:

1. Palestinian blue ID card with 2 photocopies.
2. UNRWA refugee card with 2 photocopies.
3. Actual travel document with 1 photocopy.
4- The 2 fingerprinted Palestinian identification cards.
5. 4 photos (Passport size).
6. Canadian Landed Immigrant paper.
7- The amount of:

40 CAN$ for one year
120 CAN$ for three years
200 CAN$ for five years
In person: Cash.
By mail: Money order payable to the Embassy of Lebanon.

8- Self-addressed stamped, Express Post, envelope with phone number.
